What is Skilled Nursing Services?
Home skilled nursing services is an in-home service care that is ordered by a physician, called skilled care. Visits are typically one to five times weekly and can be extended visits up to 24/7 skilled care.

The type of care ordered can require more than one discipline to help in reaching the optimal level of progress reaching health goals

Home skilled nursing services may be a combination of nurses, nursing assistants, and therapists. Care is very diverse and will be discussed with you at the time of admission

What to expect
Pam Health at Home will receive a referral or an order from your doctor for home health care services. We will then contact you for additional information and set up an appointment.

A Registered Nurse, Physical Therapist, or Speech Therapist will complete a comprehensive assessment, reviewing your medical history, current health and medications. You will sign a Consent to Treat form. Please feel free to ask the clinician questions

The clinician may want to observe your mobility in the event you need safety devices. The clinician will also complete a safety assessment of your home and make suggestions if any are needed.

The clinician will create a calendar of visits that works best for your schedule. She will provide and review your consumer rights, privacy rights, and will give you information regarding advance directives.
